1. I received an administrative subpoena and I can’t locate my tax documents to comply with the request to file.  How can I obtain my tax information?

You may contact the IRS, the State of Ohio, and your employer(s) and request they provide you with copies of the returns you may have filed and the W-2’s you were issued.

The State of Ohio Department of Taxation can be contacted at 1-800-282-1780 (Individuals), 1-800-282-1784 (Refund Status), 1-800-282-1780 (School District Tax), or 1-888-405-4089 (Businesses).  The State of Ohio Department of Taxation website is http://tax.ohio.gov.

  1. How can I contact the Internal Revenue Service (IRS)?

The IRS can be contacted at 1-800-829-1040 (Individuals), 1-800-829-4477 (Refund Status), or 1-800-829-4933 (Businesses).  The IRS website is www.irs.gov.

  1. Will RITA accept photo-copies of my W-2’s? 

Yes.  Legible photo-copies are acceptable.

  1. If I provide RITA the complete tax information requested to fulfill the administrative subpoena request requirements, do I still need to appear at the location and scheduled time as indicated on the subpoena?

No, the request for the specified documents has been satisfied, there is no reason to appear.

  1. If I want to provide the information prior to my scheduled date, where would I send it?

  1. I cannot locate my W-2’s.  Can I provide a copy of my federal tax return?

A federal tax return will be accepted; unfortunately it does not display the municipal tax that was withheld for your work place city. Without this figure you will not receive credit for the withholding when your residence tax is calculated

  1. What will happen if I appear at the subpoena program and file my tax returns but I am unable to pay?

You will receive a billing statement shortly after your appearance.  You also have the option of setting up monthly payment arrangements which will freeze your penalty and interest

  1. What type of payments will be accepted at the subpoena program?

You will be able to pay by credit card; we accept Visa, Master Card and Discover.  We also accept checks and money orders.  Sorry no cash.

  1. I received a subpoena from the city requesting that I appear in Mayor’s Court.  Can I send my information to RITA?

No, you must appear in Mayor’s Court with your information.
